1. Purpose
This clause outlines the consent requirements and conditions applicable where a patient’s personal information is used for public fundraising campaigns on the Medfund platform and associated channels.
2. Scope of Application
- This provision applies to any Patient for whom a fundraising campaign is created on the Medfund platform.
- Where the Patient is a minor or otherwise legally incapable of providing consent, this clause applies to the Patient’s lawful guardian, caregiver, or legally authorized representative.
3. Consent to Collection and Processing
By approving the creation of a fundraising campaign, the Patient or their lawful guardian/authorized representative expressly consents to the collection, processing, and use of personal information necessary for the purpose of fundraising.
Such personal information may include, but is not limited to:
- Full name of the Patient
- Medical condition and relevant health information
- Photographs
- Video recordings
- Medical reports or supporting documentation
- Treatment updates and progress reports
4. Consent to Public Disclosure
The Patient or their lawful guardian/authorized representative acknowledges and agrees that the above information may be published and shared publicly for the sole purpose of facilitating and promoting fundraising efforts.
Such publication may occur on:
- The Medfund platform
- Medfund’s official social media pages
- Third-party social media platforms
- Digital advertising platforms
- Promotional materials related to the fundraising campaign
5. Acknowledgment of Public Nature of Information
The Patient or their authorized representative understands and acknowledges that:
- Once information is shared publicly, it may be viewed, shared, reposted, or redistributed by third parties.
- Medfund cannot fully control or prevent further dissemination once the information is publicly available.
6. Authority and Accuracy
By providing consent, the Patient or authorized representative confirms that:
- The information provided is accurate and truthful to the best of their knowledge.
- They have the legal authority to provide such consent on behalf of the Patient where applicable.
7. Limitation of Use
Medfund shall publish only information reasonably necessary for fundraising purposes and shall process all personal data in accordance with applicable data protection and privacy laws.
